All insurance programs have a deadline for submission of injury claims. These deadlines range from 30 to 90 days AFTER the accident.
If possible, have any attending physician (or family physician) fill out the necessary portion of the injury form.
These insurance programs are considered SECONDARY coverage, meaning they only apply if your primary insurance coverage has been exhausted or if your primary insurer has declined your claim for Athletic Therapy treatments (your primary insurer will provide a letter stating this, which may be required for coverage with these secondary insurers)
